Packages - HCP
The CHSP is complemented by the HCP program which provides the second tier of support in the aged care system. The HCP program is designed to support older people living in the community whose care needs exceed the level of support provided through the CHSP. It provides consumers with higher intensity, ongoing services and case management as well as an individualised budget developed by the consumer and their provider and sets out how available package funds will be used to deliver the care and services the consumer needs. The HCP program provides four levels of consumer directed coordinated packages of services for more complex support for older people who are able to continue living independently in their own homes with assistance.

Assessment for a
Home Care Package
In order to be approved for the package an assessment by an Aged Care Assessment Team (ACAT) will need to take place. During the assessment, the ACAT will determine eligibility for a home care package and which package level best meets the care needs. There are four levels:
- Level 1 – basic care.
- Level 2 – low-level care needs.
- Level 3 – intermediate care needs.
- Level 4 – high-level care needs.
Each level of home care packages provides a different subsidy amount. This amount is paid to the selected approved home care provider. The subsidy contributes to the total cost of services and care delivery, with the Consumer expected to contribute where personal circumstances allow.

Each consumer will have an individually tailored package of care specifically designed to support the consumer to meet their goals, needs and preferences. Home Care Package will be used to provide a wide range of services including support services, personal care, nursing care, allied health and other clinical services, care coordination and case management. HACS will work in partnership with their clients to understand what they want and hope to achieve, and then deliver services that meet those needs. HACS will follow the following process to ensure the client receives the minimum requirements from a CDC provider:
- A Home Care Agreementmust be offered to each client. The Agreement records the administrative details of the service delivery relationship.
- A written care planwill be provided to the client within 14 calendar days of entering into a Home Care Agreement, which sets out the day-to-day services that will be provided and how they will be delivered.
- An Individualised budgetwill be provided to the client that records the funds available in their home care package (including Government subsidies and supplements and fees or contributions paid by the client) and how they will be used.
- A Monthly statementmust be provided to the client outlining the available funds and expenditure for their package in the relevant month.
Providing services to clients who have Home Care Packages through Care/Package management and services delivery is the goal. HACS prefers to manage Home Care Packages for clients while charging a care and package management fee. However clients with self- managed packages can be serviced under private paying clients for services.
When working with Clients to decide the care services and supports in their care plan, HACS Care Managers will:
Identify personal health and care needs and the type of services that the client wants in order to support them in their daily life.
Identify the amount of hours available to spend in the care budget and the rates the client would like to pay.
Identify what skilled and qualified care workers the client will need

HACS will provide a dedicated team to provide holistic consumer directed care working with the dedicated Home Care Coordinator in conjunction with qualified nursing staff and contractor allied health professionals to ensure clients’ needs are met. The dedicated HCC will make contact with potential clients and guide them throughout the client journey from sign-up, planning and coordination of required services, providing ongoing monitoring and review of services and claiming of funds for services provided from the department. The client will be charged a Care and Package Management Fee for administration of these services. All services will be provided in compliance to ACQSC standards.
Self-
Managed
Hola Aged Care Services will support consumers to self-manage their home care packages with practical direction, guidance, and education, ensuring they are able to manage their own Home Care Packages on their own and continue to live independent, healthy, and supported lives.
The self-managed workforce predominantly consists of clinical and administrative support and care managers that set up and support clients. Service delivery will incorporate easy and readily available access to home care services. HACS will design the care plan and budget with the client and then assist the client to go directly to their personal carers, clinical and support workers. HACS aims to ensure the client will feel confident managing their own package, and will provide the tools to do so.

HACS will support and provide different home services based on clients’ wishes and clinical indications including but not limited to the following:
Personal services: assistance with personal activities such as bathing, showering, toileting, dressing and undressing, mobility and communication.
Nutrition, hydration, meal preparation and diet: assistance with preparing meals, including special diets for health, religious, cultural or other reasons; assistance with using eating utensils and assistance with feeding.
Continence management: assistance in using continence aids and appliances such as disposable pads and absorbent aids, commode chairs, bedpans and urinals.
Nursing interventions and services: In home qualified nursing services helping managing wounds, medication, continence and more. They can carry out health assessments and reviews, and help with monitoring blood pressure, oxygen levels and other general wellbeing needs. Nursing will also support seniors who are recovering from injury or illness and need some help to get back on their feet. They will be able to develop individual nursing assessments and care plans, skin integrity and wound-care management, continence management and medication supervision and management. Nursing services will be offered on a 1-on-1 basis only.
Transport and personal assistance: assistance with shopping, visiting health practitioners and attending social activities.
Externally provided services
Facilitation, support and assistance of Consumers receiving: allied health and other clinical services e.g. podiatry, occupational or physiotherapy services, hearing and vision services while under core-primary service agreements.
Allied health therapy – occupational therapy, physiotherapy, massage therapy, dietetics, etc.
HACS’s care team will be highly experienced in Aged Care leadership, clinical nursing and case management, financial management and recruitment. The leadership team are involved in other government initiatives and bring experience in providing package management services for government funded disability schemes.
